Cat Terms of
Adoption Contract
This ADOPTION AGREEMENT is entered into this_____ day of
________, 20___, between Eastern Montana Humane Society,
Inc., (hereinafter referred to as EMHS)
and_______________________________, (hereinafter
referred to as “Adopter”).
1.___INDOORS: My pet will be kept securely indoors at
all times. Reason: Indoor cats have a longer life span
because of a lower risk of disease, injury, abuse and
death by automobile.
2.___NO DECLAWING: I will not have my cat declawed. I
will use alternative methods such as nail trimming,
appropriate scratching posts or Soft Paws plastic nail
covers. Reason: Declawing is no guarantee claws won’t
grow back, often leads to behavioral problems, and
causes great pain through amputation of bones, as well
as tissue.
3.___LOST PET: Should my pet become lost, I will call
EMHS immediately at 406-778-2436 or 406-778-3898. In
addition to contacting EMHS, I will also contact Animal
Control (406-778-2151). To help prevent my pet becoming
lost, I agree to provide a safe collar with a rabies tag
and an ID tag with my name and phone number.
4.___PERMANENT FAMILY MEMBER: I am adopting this animal
as a permanent member of my family and if for any reason
I cannot keep my pet, I will return my cat to EMHS. I
will not relinquish my pet to any shelter, society or
person other than EMHS. Adopter expressly covenants and
agrees that they will not euthanasia the pet without
prior approval from EMHS.
5.___CARE OF PET: I agree to provide my pet with a safe,
clean and comfortable living environment, with adequate
water, food, ventilation and affection at all times. I
agree to feed only high quality food, preferably Science
Diet. Reason: Low quality food can cause diarrhea as
well as increased risk of weight gain, and kidney, live,
and heart diseases. Because EMHS is concerned for the
welfare of adopted animals and the adopters, I agree to
follow-up visits and/or calls from an authorized EMHS
representative.
6.___VETERINARY CARE: I will provide veterinary care for
my pet, including yearly vaccinations/checkups, and
treatment for any illnesses. Should my cat become ill, I
understand its medical and dental care will be my
financial obligation. I understand that EMHS has
provided all age-appropriate vaccinations and found no
health problems through veterinary examination, unless
noted on health record. I understand that in spite of
efforts to ensure optimal health ,it is impossible to
guarantee “perfect health” in an animal.
7.___SPAY/NEUTER: I agree to prevent offspring by
spaying/neutering the pet by ___________, I understand
that the adoption is conditional and not final until the
pet is spayed/neutered. I will send a copy of my receipt
to EMHS as soon as surgery is performed.
8.___BEHAVIORAL PROBLEMS: Because adjustment to a new
home and family can be difficult for my cat, I will call
EMHS immediately if I see any behavioral problems
develop. By getting help quickly, I have a better chance
of deterring unwanted behavior.
9.___HONOR CONTRACT: I hereby bind myself to the
fulfillment of the above clauses and realize that
failure to comply with these clauses may result in
repossession of the cat and legal action. I attest that
the information I’ve provided to EMHS on this contract
is truthful and accurate.
10.___Adopter will notify EMHS at least 1 week prior to
any change of address, phone number, etc., of the
adopter.
LIMITED DAMAGES: I agree to pay EMHS the sum of $200 as
liquidated damages in the event the terms of the
contract are beached. This liquidated damaged value is
agree to for the purpose of establishing value of the
animal and does not bar EMHS from seeking return of the
animal by judicial process or other legal means.
ATTORNEY’S FEES/COURT COSTS: I hereby accept possession
of title to ( subject to the conditions of this
contract), and responsibility for the animal named below
and hereby release and discharge EMHS from liability for
any injury or damages to any person or property caused
in the future by said animal, and from any causes of
action, claims, suits or demands whatsoever that may
arise as a result of such injury or damages. I agree to
be governed by the laws of Montana in all matters
relating to this animal and release, hold harmless, and
agree to indemnify EMHS from any liability for
injury/damage caused by this animal. This is the entire
agreement between EMHS and me.

Dog Terms of Adoption Contract

We agree that the pet will not be left outside
unattended at any time.
3.___PICKUPS: At no time will the dog be allowed to ride
in the open bed of a pickup truck.
